DM can increase the risk of chronic conditions such as visual impairment, lower extremities problems affecting mobility, and other functional limitation related to cardiovascular diseases. Our nurses will assess your abilities in self-monitoring blood glucose, administration of oral medications, and insulin injection techniques. We can help you answer your questions and give the right tools to manage your condition.

Continuity Nursing Care can assist in setting goals with you and your family to encourage you to become an active participants in the management of the DM regimen; to experience few or no episodes of hyperglycemia or hypoglycemia emergencies; to maintain blood glucose level at normal or near-normal levels; to prevent, minimize, or delay the occurrence of chronic complications of DM and to adjust the lifestyle to accommodate DM regimen with minimal stress.
