Continuity Nursing Care believe in prevention being the best treatment for pressure ulcer and we can provide the education for both the client and family. It is important to perform risk assessment with a validated tool to determine how aggressive the preventive measure should be.
Our team of qualified nurses will assess and treating wounds with various etiology and create a care plan for clients with acute or chronic wounds. We will start by collecting a comprehensive patient history; understand aetiology of the wound; determine a treatment plan and assess progress of the wound. We work with clients who have acute, chronic, surgical (post-hospital discharge), traumatic, abscesses, diabetic ulcer, venous and arterial ulcers. Our nurses also have extensive experience working with Negative-Pressure Wound Therapy to help manage wounds with various nature such as dehisced wounds, partial-thickness burns, ulcers, flaps and grafts. We can assist in managing all type of wound by assessing and re-assessing until a satisfactory outcome has been reached.
