To offer compassion and support to people of all ages in the Ottawa Ontario region.  Continuity Nursing Care provides nursing services for the client to stay at home as long as possible, from wound care management by the nurse to assistance with bathing, eating, companionship, and housekeeping services by our Personal Support Workers. 

    In-Home Care Services

    We alleviate the caregiver burden and provide customized in-home services for you or your family member, so you can have peace of mind knowing they are being well taken care of, every day. Our personal care services give independence to our clients while ensuring they are healthy and well cared for. Continuity Nursing Care is rooted in building a warm, therapeutic client relationship, while maintaining a healthy, positive working environment.

    Our Services

    We work with clients who have chronic illnesses such as Arthritis, Chronic Obstructive Pulmonary Disease (COPD), Alzheimer’s Disease and other Dementias, Multiple Sclerosis, Parkinson’s, Cancer, Stroke, Heart Disease and Diabetes.  We offer palliative and hospice care, wound care post hospital discharge, mental health support, respite, and assistance with other needs. Our mission is to improve quality of life for all Canadians in every aspect of their daily life so they can age at home.

    Instrumental Activities of Daily Living (IADLs).

    Our Process

    Once we have a request for services, we will meet with you to see what your needs are. Then we will discuss the care options and see what's the best fit for you or your family member. Once a Care Package has been chosen with the client and family consent, we can start the admission process. Our detailed and thorough admission process will assess every aspect of our client to maximize the care we provide. The care plan will be reviewed regularly by our nursing staff and changes will be made to better support our client needs.

    Full Care Package

    The Full Care Package is designed for clients who require 4 hours of personal assistance with care in their home with ADLs and IADLs.

    Home Care Plus Package

    Home Care Plus Package is designed for clients who need 8 hours of personal assistance with care in their home with ADLs and IADLs.

    Continuous Care Package

    Continuous Care Package is designed for clients who need 24-hours of personal assistance with care in their home with ADLs and IADLs.

    Live-In Care Package

    Live-In Care Package is designed for clients who need 24-hours of personal assistance with care in their home with ADLs and IADLs.
